Formal application for access to government information held by any public sector agency
Use this form when making a formal application to access government information held by a NSW public sector agency, other than the IPC. Do not send this form to the IPC.

Apply for an external review by the Information Commissioner
Use this form if you are applying for an external review by the Information Commissioner of a reviewable decision under the Government Information (Public Access) Act 2009 (GIPA Act).
Make a complaint to the Information Commissioner about the conduct of an agency under the GIPA Act
Use this form if your complaint concerns the conduct (action or inaction) of an agency in exercising its function under the Government Information (Public Access) Act 2009 (GIPA Act).
As stipulated in the IPC’s Privacy Policy, the IPC ensures information collected complies with the Privacy and Personal Information Protection Act (PPIP Act) and the Health Records and Information Privacy Act (HRIP Act). |
